Nutrition Rx Program
Revitalize Community Development Corporation, Baystate/HNE - BeHealthy, Community
Care Cooperative (C3), WellSense Boston Children's, Wellsense Mercy Alliance, Wellsense Southcoast Health Accountable Care Organizations (ACOs) have joined forces to provide support to individuals experiencing very low food security meet their nutritional needs.
The Nutrition RX Program provides medically tailored or nutritionally appropriate grocery items curated by a Registered Dietitian Nutritionist. Individuals will receive a monthly grocery box, delivered to their home or a food voucher. Other available services include kitchen supplies, nutrition counseling, and group classes.

Participants must meet the specific criteria of the MassHealth Health Related Social Needs program.

Participants may be eligible for kitchen supplies such as pots and pans, food preparation items or small appliances based on individual need.

Nutrition Counseling provided by a Registered Dietitian Nutritionist is available for all participants.

Group classes are held monthly with discussion and recipe demonstration.

Groceries are chosen following the Dietary Guidelines for Americans and medically tailored to meet individual needs and goals.
